Opportunity for impact
We are seeking dedicated NPs to join our growing pods at Auxira Health. In this role, you will provide remote cardiovascular care to patients on behalf of our clients, working closely with their physicians and clinical teams across multiple geographies and offices within a virtual medicine practice. You will collaborate with multidisciplinary teams to deliver high-quality, personalized care, ensuring seamless coordination of care, top-of-license service, and optimal patient outcomes.
Role and responsibilities
- Deliver telehealth services in collaboration with our clients' physicians to manage low-acuity cardiovascular disease patients across the nation.
- Provide virtual patient assessments, coordinate care, and monitor patient progress remotely.
- Collaborate with in-office clinicians and staff at our client practices to ensure continuity of care.
- Utilize technology platforms and workflows designed to optimize patient care delivery and streamline workloads for our clients' clinicians.
- Ensure timely and accurate documentation of patient encounters in electronic medical records (EMRs), in line with our client practices' standards.
- Educate patients and their families on cardiovascular conditions and the importance of lifestyle modifications.
- Participate in ongoing training and continuing medical education (CME) to ensure excellence in service delivery and professional growth.
Professional qualifications
- Certified Nurse Practitioner (NP), licensed in current partner states — subject to change as our network of partnerships grows.
- A minimum of 3 years of experience as an NP in cardiovascular care, with outpatient clinic experience.
- Strong understanding of telehealth platforms and remote patient monitoring.
- A dedicated service mindset, to provide exceptional support to our partner cardiologists.
- Excellent communication and interpersonal skills, with the ability to collaborate closely with physicians and clinical staff.
- Commitment to providing compassionate and patient-centered care.
- Ability to work independently in a remote environment while maintaining close coordination with multidisciplinary teams.
- Ability to travel to our partner cardiology clinics during the first four weeks for training.
The ideal candidate also has
- Prior experience in telehealth or remote care delivery.
- Outpatient clinic experience in the heart failure subspecialty.
- Dual licensing in multiple states (highly desirable).
